The clinic has some English content on their website, which is a decent sign, but “some English online” doesn’t always translate to smooth English service at the front desk. Walk-in communication could be hit or miss. If your Japanese is nonexistent, bring a translation app or a bilingual friend for anything beyond a routine eye exam.
