Their website has some English content, which is a decent sign, but don’t take that as confirmation that your appointment will go smoothly in English. The reality is probably closer to basic communication with some Japanese required. A translation app would be smart to bring. That said, internal medicine clinics like this are generally good at handling straightforward health concerns without a lot of back-and-forth complexity.

Suginami is a residential ward — not a foreigner-heavy area like Shinjuku or Minato — so don’t expect an internationally-oriented setup. But if you live nearby and need a reliable local GP equivalent, this is worth knowing about.
