English is genuinely functional here, not just a checkbox. Reception can handle basic conversation, and the clinical staff can communicate well enough to get through an appointment without you needing to pull out Google Translate every five minutes. That said, if you’ve got a complex medical history or very specific concerns, bringing a few written notes in Japanese doesn’t hurt. Forms may still be primarily in Japanese.
