Japanese Clinic Language Guide for Expats

Introduction

Visiting a doctor in Japan can feel overwhelming when you are not fluent in Japanese. Most clinics outside major international cities have limited English support, and receptionists and doctors will often communicate primarily in Japanese. This guide gives you practical, ready-to-use phrases to help you check in, describe your symptoms, explain your medical history, and understand what comes next. Each phrase is written in Japanese script, romanized Japanese (romaji), and English so you can point at the page or practice before your visit. Even a basic attempt to speak Japanese is greatly appreciated by medical staff and can help your appointment go more smoothly.

Checking In at Reception

When you arrive at a Japanese clinic (クリニック, kurinikku) or hospital (病院, byouin), go to the reception desk (受付, uketsuke) and follow these steps.

  • First visit:

    初めて来ました。
    Hajimete kimashita.
    This is my first visit.
  • Returning patient:

    以前にも来たことがあります。
    Izen ni mo kita koto ga arimasu.
    I have been here before.
  • Presenting your health insurance card:

    保険証です。
    Hokenshou desu.
    Here is my insurance card.
  • If you do not have Japanese insurance:

    保険証を持っていません。自費で支払います。
    Hokenshou wo motte imasen. Jihi de haraimasu.
    I do not have an insurance card. I will pay out of pocket.
  • Asking for an English form or interpreter:

    英語の問診票はありますか?
    Eigo no monshinpyou wa arimasu ka?
    Do you have an English medical questionnaire?
    英語を話せる人はいますか?
    Eigo wo hanaseru hito wa imasu ka?
    Is there someone who speaks English?
  • Stating why you are there generally:

    具合が悪いです。
    Guai ga warui desu.
    I am not feeling well.

Common Symptom Phrases

Fever

  • 熱があります。
    Netsu ga arimasu.
    I have a fever.
  • 体温が38度あります。
    Taion ga sanjuuhachi-do arimasu.
    My temperature is 38 degrees Celsius.
  • 悪寒がします。
    Okan ga shimasu.
    I have chills.

Pain

  • 痛いです。
    Itai desu.
    It hurts. / I am in pain.
  • 〇〇が痛いです。
    〇〇 ga itai desu.
    My [body part] hurts.
  • 足が痛いです。
    Ashi ga itai desu.
    My leg/foot hurts.
  • 胸が痛いです。
    Mune ga itai desu.
    I have chest pain.
  • 背中が痛いです。
    Senaka ga itai desu.
    My back hurts.

Nausea

  • 吐き気がします。
    Hakike ga shimasu.
    I feel nauseous.
  • 嘔吐しました。
    Outo shimashita.
    I vomited.
  • 気分が悪いです。
    Kibun ga warui desu.
    I feel unwell / I feel sick.

Headache

  • 頭が痛いです。
    Atama ga itai desu.
    I have a headache.
  • 頭痛がひどいです。
    Zutsuu ga hidoi desu.
    I have a severe headache.
  • めまいがします。
    Memai ga shimasu.
    I feel dizzy.

Stomach Problems

  • お腹が痛いです。
    Onaka ga itai desu.
    I have stomach pain / a stomachache.
  • 下痢があります。
    Geri ga arimasu.
    I have diarrhea.
  • 便秘です。
    Benpi desu.
    I am constipated.
  • 食欲がありません。
    Shokuyoku ga arimasen.
    I have no appetite.

Throat and Respiratory

  • 喉が痛いです。
    Nodo ga itai desu.
    My throat hurts / I have a sore throat.
  • 咳が出ます。
    Seki ga demasu.
    I have a cough.
  • 鼻水が出ます。
    Hanamizu ga demasu.
    I have a runny nose.
  • 息苦しいです。
    Ikigurushii desu.
    I have difficulty breathing.

Rash and Skin

  • 発疹が出ています。
    Hasshin ga dete imasu.
    I have a rash.
  • かゆいです。
    Kayui desu.
    It is itchy.
  • 皮膚が赤くなっています。
    Hifu ga akaku natte imasu.
    My skin is red.
  • 腫れています。
    Harete imasu.
    It is swollen.

Fatigue

  • 疲れています。
    Tsukarete imasu.
    I am tired / fatigued.
  • だるいです。
    Darui desu.
    I feel sluggish / I have no energy.
  • 最近ずっと疲れています。
    Saikin zutto tsukarete imasu.
    I have been feeling tired lately.

Describing Pain Intensity and Duration

Intensity

  • 少し痛いです。
    Sukoshi itai desu.
    It hurts a little.
  • かなり痛いです。
    Kanari itai desu.
    It hurts quite a lot.
  • とても痛いです。
    Totemo itai desu.
    It hurts very much.
  • 我慢できないほど痛いです。
    Gaman dekinai hodo itai desu.
    The pain is unbearable.
  • You may also be asked to rate pain on a scale. Say:

    10段階で〇です。
    Juudan-kai de [number] desu.
    On a scale of 10, it is [number].

Type of Pain

  • ズキズキします。
    Zukizuki shimasu.
    It is throbbing.
  • 鋭い痛みです。
    Surudoi itami desu.
    It is a sharp pain.
  • 鈍い痛みです。
    Nibui itami desu.
    It is a dull ache.
  • 焼けるような痛みです。
    Yakeru you na itami desu.
    It is a burning pain.

Duration

  • 今日から痛いです。
    Kyou kara itai desu.
    It has been hurting since today.
  • 昨日から痛いです。
    Kinou kara itai desu.
    It has been hurting since yesterday.
  • 3日前から痛いです。
    Mikkamae kara itai desu.
    It has been hurting for three days.
  • 1週間ほど続いています。
    Isshukan hodo tsuzuite imasu.
    It has been going on for about one week.
  • 時々痛みます。
    Tokidoki itamimasu.
    It hurts occasionally.
  • ずっと痛いです。
    Zutto itai desu.
    It hurts constantly.

Saying You Have Allergies or Take Medication

Allergies

  • アレルギーがあります。
    Arerugii ga arimasu.
    I have allergies.
  • 〇〇にアレルギーがあります。
    〇〇 ni arerugii ga arimasu.
    I am allergic to [substance].
  • ペニシリンにアレルギーがあります。
    Penishirin ni arerugii ga arimasu.
    I am allergic to penicillin.
  • 薬にアレルギーがあります。
    Kusuri ni arerugii ga arimasu.
    I have a drug allergy.
  • 花粉症があります。
    Kafunshou ga arimasu.
    I have hay fever / pollen allergy.
  • 食物アレルギーがあります。
    Shokubutsu arerugii ga arimasu.
    I have a food allergy.

Current Medications

  • 薬を飲んでいます。
    Kusuri wo nonde imasu.
    I am taking medication.
  • これが私が飲んでいる薬のリストです。
    Kore ga watashi ga nonde iru kusuri no risuto desu.
    This is a list of the medications I am taking.
  • Tip: Bring your actual medication packaging or write the name clearly in English and Japanese if possible. You can also show the bottle.

  • 毎日〇〇を飲んでいます。
    Mainichi 〇〇 wo nonde imasu.
    I take [medication name] every day.
  • 持病があります。
    Jibyou ga arimasu.
    I have a chronic condition / pre-existing illness.
  • 糖尿病があります。
    Tounyoubyou ga arimasu.
    I have diabetes.
  • 高血圧があります。
    Kouketsuatsu ga arimasu.
    I have high blood pressure.
  • 妊娠しています。
    Ninshin shite imasu.
    I am pregnant.
