  • Dermatology and internal medicine combined
  • Near central Tokyo
  • TSAI Clinic sits in Hatsudai, just west of Shibuya — close enough to be convenient, quiet enough to not feel chaotic. It’s a combined internal medicine and dermatology practice run under the name Sai Naika Hifuka Clinic, with dermatology being the main draw for most expats. The clinic has some English content on its website, which is a decent sign, but don’t assume you’ll walk in and have a fully English consultation waiting for you. Reality is probably somewhere in the middle — a doctor who can manage basic English, reception that may or may not. Bring a translation app as backup. For skin concerns like acne, eczema, rashes, or anything you’d rather not leave untreated while navigating a language barrier, this is a reasonable option in the Shibuya area. Not a dedicated expat clinic, but not a wall of zero English either.

    Patient Feedback

Patient feedback points to a thorough, no-rush approach from the doctors — people don’t feel rushed out the door. Wait times can stretch, especially without an appointment. The clinic has a calm, neighborhood-practice feel rather than a high-volume urban mill. Dermatology patients generally report clear explanations of treatment plans. It’s not flashy, but it’s competent and consistent, which is what you actually want when your skin is doing something alarming.

    English Language Proficiency

The website has some English sections, which suggests at least some awareness of non-Japanese patients — but that doesn't guarantee a smooth English consultation in person. Reception English is unconfirmed. The doctor may have functional English, but go in prepared. Load up Google Translate or Midori, screenshot your symptoms in Japanese beforehand, and you'll be fine. Forms are almost certainly Japanese-only. This isn't a clinic built around expat care, but it's not a hard no either.
